- How to Enter: Visit the CVS Home Team Heroes website, complete the online entry form by nominating a child, answering three questions (250 words or less each) about the child’s community involvement and soccer experience, and upload three photos (2 soccer participation, 1 community involvement).
- Eligibility: Nominators must be legal residents of the 50 United States and the District of Columbia, eighteen (18) years of age or older as of date of entry, who are the parent or legal guardian of a child seven (7) to fifteen (15) years of age at the time of entry who are active soccer players and are members of a tax-exempt nonprofit soccer organization. Nominees must be the child/charge of the Nominator, a legal resident of the 50 United States and the District of Columbia, seven (7) to fifteen (15) years of age as of date of entry, and an active soccer player.
