Alternatively, enter by calling 1-855-855-5905 to complete an automated customer satisfaction survey, or mail in your entry with personal details on a 3×5-inch piece of paper.
- Eligibility: Open to legal residents of the 50 United States and the District of Columbia. Entrants must be 18 years of age or older at the time of entry, except for residents of Alabama and Nebraska who must be 19 years or older. Walgreens employees and their immediate family members living in the same household are not eligible.
